Barry Keoghan is once again at the center of attention—not for a new role, but for his striking transformation that has left fans and experts speculating. Since his early days in Between the Canals back in 2010, the Irish actor appears to have undergone a noticeable evolution in his appearance, with sharper facial features and a more defined overall look.
The 33-year-old star, recently seen at the premiere of The Peaky Blinders: The Immortal Man in New York and at the Oscars, has sparked widespread discussion online. Many have pointed out his more sculpted jawline and fuller lips, fueling rumors that he may have opted for subtle cosmetic enhancements rather than dramatic procedures.

At the same time, Keoghan has also been linked to one of the most iconic roles in cinema history—James Bond. However, the actor himself was quick to shut down those rumors, offering a candid assessment of his suitability for the role.
“James Bond is an iconic role and a lot of weight and pressure comes with that,” he told Radio Times. “It's nice to see your name go up there, but I don't think I fit the criteria for James Bond. I'd rather come in and do the villain. The man teasing Bond, that's more me.”
While Keoghan has neither confirmed nor denied undergoing any cosmetic procedures, experts suggest that his transformation could be the result of a series of subtle aesthetic treatments. Dr. Richard Devine, an aesthetic doctor and founder of the Devine Clinic in London, offered his professional insight after analyzing the actor’s recent appearances.
“When you compare recent photos of Barry Keoghan to earlier images from the start of his career, his facial structure appears noticeably sharper and more defined,” Dr. Devine explained. “The jawline looks more sculpted and the cheekbones appear slightly more prominent. In many cases, this kind of change can be achieved through a combination of subtle aesthetic treatments rather than dramatic surgical procedures.”
He further elaborated on how such results are typically achieved, noting: “Facial masculinisation treatments are designed to enhance the natural angles of the face. In men, that often means strengthening the jawline, adding subtle projection to the chin and emphasising the cheekbone structure to create a more sculpted, angular appearance.”

According to Dr. Devine, non-invasive methods can play a significant role in shaping facial features. “Small amounts of dermal filler placed along the jawline or chin can enhance natural bone structure and create a stronger, more masculine profile. When performed conservatively, the goal isn’t to change someone’s face, but to emphasise angles that already exist.”
In addition to fillers, other modern treatments may contribute to the refined look. “We also increasingly use collagen-stimulating treatments and skin-tightening technologies such as laser resurfacing or radiofrequency-based devices,” he said. “By tightening the skin and improving collagen production, the underlying bone structure becomes more visible, which naturally gives the face a sharper, more chiselled look.”
The actor’s lips have also drawn attention, appearing fuller in recent images. Dr. Devine noted: “Barry Keoghan’s lips appear noticeably fuller and more defined in recent photos compared to earlier images from the start of his career. This kind of subtle change is commonly achieved with small amounts of lip filler used to enhance shape and hydration rather than dramatically increase size.”
However, he was careful to point out the limitations of such observations: “Of course, without treating the individual directly it’s impossible to say exactly what treatments, if any, have been performed. But the combination of natural maturation, lifestyle changes and subtle aesthetic treatments can certainly produce the kind of facial refinement we’re seeing.”
He also highlighted the delicate balance required in male aesthetic procedures: “Looking at some of the most recent images from 2026, his face appears slightly fuller through the mid-cheek area compared to earlier photos. The cheeks look a little more rounded, which can sometimes suggest filler placed in the anterior cheek. In men, over-projecting this area can soften facial angles and slightly feminise the face, which is why practitioners usually approach cheek augmentation very conservatively in male patients.”

Despite the speculation, Keoghan’s transformation could just as easily be attributed to natural factors. As Dr. Devine concluded: “The combination of natural maturation, lifestyle changes and subtle aesthetic treatments can certainly produce the kind of facial refinement we’re seeing.”
